WITC Transcripts

To request a copy of your WITC Transcript, please:

• Download WITC Transcript Form   (pdf form requires Adobe Reader to view and complete)
   
• Either print and complete the Transcript Request Form OR key the information directly into the form and save to your computer
   
• Complete and sign the printed form OR complete the fill-in form and electronically sign it
   
• Mail or fax the completed Transcript Request Form to the WITC location you are requesting transcripts from OR email the completed and electronically signed form

Or if you have access to MyWITC (your personal portal), you may request a transcript using this online service.

NOTE:  There is no charge for a WITC Transcript
